This role will function within the interdisciplinary medical team, supporting and coordinating the mental health and physical health of patients on an assigned patient caseload. The BHCM will be responsible for outreach and engagement of the assigned patient caseload, providing care and resource coordination. The BHCM will utilize a registry in order to track treatment outcomes, trends, and summarize treatment efforts. Through engagement, the BHCM will develop a therapeutic relationship, provide assessment, treatment care planning of mental health and substance use disorders, and administration of validated rating scales.
Minimum: Bachelor's Degree in Behavioral Health Field (e.g. Clinical Social Work, Psychology) AND at least 2 years of experience working with individuals with mental health and substance use disorders. Preference: Master's Degree in Behavioral Health Field (e.g. Clinical Social Work, Psychology) Required: Fluent in Spanish. Must be a strong communicator in person and via phone, have experience within multi-disciplinary team, and the ability to develop rapport quickly.

Requires frequent sitting and standing. Operation of standard office machines and computer. May require lifting of up to 25 pounds. Requires eye-hand coordination and manual dexterity. Requires normal vision range. Work is performed in an office environment. Work in multiple clinics required.
