Job Summary

The primary responsibility of the RN Care Manager is assess, plan, implement, coordinate, monitor and evaluate the Medicaid Management of Care for the Pediatric population. The RN Care Manager has the responsibility for providing direct and indirect nursing care to assigned patients. Working under the direct supervision of the Nurse Manager or designee, but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which he/she is responsible.  This position will service the following sites: Charles Drew, Burlington CHC, Scott CHC, Sylvan and Prospect Hill.

MINIMUM QUALIFICATIONS

Education/Experience: Graduate of an accredited school or nursing. 1 year experience working with a diverse patient population required. Preferred: Has practiced nursing in the last 3 years; 3 years RN experience with at least 24 months experience working with Medicaid Management Care. Two years of acute care management experience is preferred and/or 1 year of community based nursing experience is required.

  • Must possess knowledge and skills necessary to treat patients
  • Meet a standardized set of competencies for the specific position 
  • Excellent interpersonal, oral and written communication skills

Type of Employees Supervised: None.

Licensure, Registry or Certification Required: RN currently licensed in NC; present credentials which demonstrate ability to meet performance criteria for the RN position; Valid NC driver's license.

Immunizations:  Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.  The Covid Vaccination is also a requirement of employment.

Diverse patient population served with a focus on Pediatrics.

PHYSICAL DEMANDS/ WORKING CONDITIONS

Requires frequent sitting for long periods, operation of standard office machines and computer.  May require lifting of up to 25 pounds.  Requires eye-hand coordination and manual dexterity.  Requires the use of office equipment, such as computer terminals, telephones and copiers.  Requires normal vision.  Work is performed in an office, hospital and home environment and exposes worker to potential environmental and health contaminants via contact with staff and patients.
