JOB SUMMARY |
Employment/schedule options: option #1 – Full-Time with benefits option #2 – 2 part time positions available working 24 hours/week with benefits.
Working under the direct supervision of the Nurse Manager or designee, the primary responsibility of the PACE Nurse is to provide direct and indirect nursing care to assigned patients by utilizing critical thinking skills to assist in the management of chronic diseases, clinical problem solving and decision making in collaboration with the IDT. This position is also responsible for evaluating patient home safety through the completion of a home care assessment. The PACE Nurse works independently in the application of advanced nursing knowledge and skills relative to NCBON scope of practice, competency and experience. Participates in call rotation, performs home visits, end of life/palliative (aka Pathways), and nursing care per nursing protocol. Completes professional performance requirements annually. |
MINIMUM QUALIFICATIONS |
Education/Experience: Graduate of an accredited school of nursing. Preferred: Has practiced nursing in the last 3 years. Prefer experience in home care or long term care setting, and ideally, PACE experience. Two years of acute care management experience is preferred and/or 1 year of community based nursing experience.
Type of Employees Supervised: None. Licensure, Registry or Certification Required: RN currently licensed in NC; present credentials which demonstrate ability to meet performance criteria for the RN position; Valid NC driver's license. Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.

Hearing: Adequate to perform job duties in person and over the telephone.
Speaking: Must be able to communicate clearly to medical providers, patients and multiple personnel in person and over the telephone.
Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens. Must meet certification by colorblind standards.
Other: Will be working with potentially hazardous chemicals, exposure to patient body fluids. Requires frequent lifting and carrying items weighing up to 40 pounds unassisted. Requires frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly throughout a regular work shift. Requires exposure to communicable diseases and/or bodily fluids. |
