JOB SUMMARY |
A Lead Care Manager will provide clinical and administrative guidance and oversight for the team of embedded care managers. The Lead Care Manager would be instrumental in providing training and expertise to the entire care management team. In addition, the Lead Care Manager would perform in the traditional embedded care manager role within a clinic. The Lead Care Manager will support the implementation and success of multiple population health programs and initiatives to improve patient health outcomes. The manager is responsible for identifying training opportunities, CM strategies and goals and policies and procedures development. The Lead Care Manager may be responsible for community collaboration and engagement as needed. |

Ethnically and racially diverse population of pediatric, adolescent, geriatric and prenatal patients. |
PHYSICAL DEMANDS/ WORKING CONDITIONS |
Extended periods of walking, standing, bending, lifting, pulling and pushing. Performance of essential duties requires close and distance vision, hearing acuity, dexterity and stamina. Must be able to perform heavy lifting, generally more than 25 pounds. Clinical environment with exposure to chemicals, pathogens and patient body fluids. Ability to work under stressful conditions. Ability to multitask. |
